We try to save teeth whenever it's reasonable to. But sometimes a tooth is too damaged to restore predictably, an impacted wisdom tooth is causing trouble, or extraction is part of an orthodontic plan. We'll be clear with you about why we're recommending extraction and what the alternatives are.

For more complex cases like impacted wisdom teeth, we coordinate with a trusted oral surgeon.

We send you home with detailed written instructions, a follow-up call within 48 hours, and a clear plan for replacing the tooth if needed, implant, bridge, partial denture, or simply leaving the space if it doesn't affect function or esthetics.

Sedation options for nervous patients

Many patients tell us their dread of extraction is about anesthesia and anxiety more than the procedure itself. We offer four sedation levels and match the option to your specific situation, anxiety, and the complexity of the case.

Local anesthesia only, the standard for simple extractions on calm patients. You're alert and comfortable; the tooth area is fully numb.

Nitrous oxide (laughing gas), added on top of local anesthesia for mild-to-moderate anxiety. You feel relaxed and slightly euphoric; effects wear off completely within 5 minutes of removing the mask, so you can drive yourself home.

Oral sedation, a prescription pill taken an hour before the appointment. Stronger relaxation, hazy memory of the visit, requires a ride home.

IV sedation, for severely anxious patients or complex cases (full bony impactions, multiple extractions). Coordinated with our board-certified anesthesiologist. You're deeply relaxed but breathing on your own; minimal memory of the procedure.

Planning for what comes next

An extracted back tooth that isn't replaced often leads to a cascade of secondary problems: the opposing tooth super-erupts (drops down with no biting partner), neighboring teeth tilt into the gap, the bone in that area gradually resorbs, and your bite slowly destabilizes. Front-tooth extractions have the same problems plus an obvious cosmetic gap.

At the extraction consultation we discuss replacement timing and options, implant (typically 3–4 months later after the bone heals), bridge (sooner, no surgery), or partial denture (least invasive). You leave knowing not just how the extraction goes but what your mouth looks like 6–12 months from now.

What to expect, step by step

Here's exactly what happens at a tooth extractions appointment at Cusp Dental, from the moment you walk in to the followup.

  1. Consultation and X-ray

    Dr. Sidhu confirms the tooth can't be saved and takes any necessary imaging (panoramic for wisdom teeth, periapical for individual teeth). We discuss replacement options before extraction so you know the next step.

  2. Anesthesia plan

    Most simple extractions use local anesthetic only. For complex or anxious patients, we offer nitrous oxide, oral sedation, or coordinate IV sedation. We never proceed until you're fully comfortable.

  3. Extraction

    The tooth is gently loosened and removed. Surgical extractions (for impacted teeth or fractured roots) involve a small gum incision and may require dissolving sutures.

  4. Hemostasis and instructions

    We pack the socket with gauze, confirm bleeding has stopped, and walk through written aftercare instructions. You leave with our after-hours number for any concerns.

  5. Followup

    We call or text the next day to check on you. Most patients heal uneventfully; if dry socket develops or pain doesn't improve, we see you back same-day at no charge.

Wisdom-tooth extractions, especially impacted lower wisdom teeth, are at the upper end of the range. Front-tooth simple extractions are at the lower end. Replacement (implant, bridge, partial) is billed separately and discussed at the consultation.

Common questions about tooth extractions

Will it hurt?

The procedure itself shouldn't, you'll feel pressure during the extraction but not sharp pain. Mild to moderate soreness for 3–5 days is typical. Take ibuprofen as directed and use ice packs the first day.

How long is recovery?

Routine extractions usually feel mostly normal within 3–7 days. Surgical extractions (impacted wisdom teeth) can take 1–2 weeks. Avoid drinking through straws, vigorous rinsing, and smoking for 72 hours to prevent dry socket.

Should I replace the tooth?

Almost always, if it's not a wisdom tooth. Empty spaces let neighboring teeth shift and the opposing tooth over-erupt, which causes bite problems years later. Your options are an implant, a bridge, or a partial, we'll talk through the trade-offs.

What is dry socket and how do I avoid it?

Dry socket is when the protective blood clot dislodges from the extraction site, exposing bone. It's painful but treatable. Avoid straws, spitting, smoking, and vigorous rinsing for the first 72 hours after extraction, that's the main prevention.
